PEF vision plans are paid for by the state or negotiating unit and are free for active employees and their families. Vision insurance is provided by Davis Vision and has a 28-day waiting period. 

How to Enroll:

To enroll in the vision plan, you must complete a Health Insurance Transaction Form (PS-404) and submit it to us.

How to Enroll:

To enroll in the vision and dental plan, you must complete a Health Insurance Transaction Form (PS-404) and submit it to the BSC.
