The Business Services Center (BSC) is currently incorporating some automation into processing employee personal data changes.
What you need to know:
- You must use the BSC Personal Data Change Form (01/25), the most current version of the Personal Data Change form (PDC), which can be found at www.bsc.ogs.ny.gov under Benefits.
- Beginning March 24, 2025, all other versions of the PDC form will be denied.
- If your form is denied, you will receive an automated email with the reason(s) it was denied.
Best Practices:
- Avoid completing the form by handwriting your information.
- Typed signatures work best but we only accept typed signatures sent from NYS work email addresses, for example [email protected].
- Addresses MUST be United States Postal Service (USPS) verified. Addresses that can’t be verified will be denied. You can verify your address is verified using the USPS Address Look Up.
- Agencies must ensure any previous versions of the form are not distributed to employees going forward.
