The Business Services Center (BSC) is currently incorporating some automation into processing employee personal data changes.

What you need to know:

• You must use the BSC Personal Data Change Form, the most current version of the Personal Data Change form (PDC).
• Beginning March 24, 2025, all other versions of the PDC form will be denied.
• If your form is denied, you will receive an automated email with the reason(s) it was denied.

Best Practices:

• Avoid completing the form by handwriting your information.
• Typed signatures work best but we only accept typed signatures sent from NYS work email addresses, for example [email protected].
• Addresses MUST be United States Postal Service (USPS) verified. Addresses that can’t be verified will be denied. You can verify your address is verified using the USPS Address Look Up.
• Agencies must ensure any previous versions of the form are not distributed to employees going forward.
